We would like to thank Dr. Monson (1) for pointing out the error in our data extraction relating to percentage lost to follow-up for his paper (2) that we included in our meta-analysis (3). Since loss to follow-up was actually less than 2 percent, the adequacy grading in table 2 of this paper should be recoded to be "Virtually complete." The corresponding value for a subsequent paper relating to the same cohort (4) may also need to be modified.

Since, as we note in the paper, most outcomes (including overall and leukemia mortality on which the effect of loss to follow-up is considered) are taken from a later paper relating to this cohort (5), for which loss to follow-up was not specified, the analyses we present are unaffected.
