This paper is an abridged version of a presentation delivered at "From Epidemiology to Policy: a Symposium on the Translation of Epidemiologic Evidence into Public Health Policy" on July 19, 1998. The symposium was jointly sponsored by the Center for Epidemiology and Policy and the Risk Sciences and Public Policy Institute of the Johns Hopkins Bloomberg School of Public Health and the American College of Preventive Medicine.
